Health Home, a person-centered program designed uniquely for each individual.
Health Home is a set of care coordination services provided by a professional care coordinator who will help you make the best use of the services and supports you currently receive. The goal of the Health Home program is to improve the coordination of care and quality, and to increase an individual’s participation in their own care. It is simply an added benefit to the clients.
What is Health Home Care Coordination?
Kin On Health Home care coordinator will work with you to receive the right care, from the right provider, at the right time and place. Health Home services include:
- Comprehensive care management – develop a personalized Health Action Plan with goals and steps to achieve those goals.
- Care coordination – improve communication with your providers, family, and other caregivers.
- Transition planning – provide support when you leave a hospital or care facility.
- Individual and family support services – identify the roles of family members, informal supports, and caregivers to help you to reach your health goals.
- Referral to community and social support services – help with activities such as transportation, food, and housing.
- Health promotion – provide education and wellness coaching to help you reach your health goals.
Who qualifies?
The program is for people with Apple Health or dual coverage with Medicaid and Medicare living with at least one qualifying chronic condition, and a PRISM score of 1.5 or higher. It is a free service to eligible clients.
