Chronic Care Management

To help clients successfully manage chronic illnesses & maintain stable & independent lives.

The Chronic Care Management Program is offered to people ages 60 or above with chronic diseases. According to the U.S. National Center for Health Statistics, a chronic disease is a disease which lasts for more than three months. Based on thorough and ongoing assessment of the client’s physical and mental needs, caregiver concerns and financial situation, the Kin On Chronic Care Management staff will provide you and your family the support you need to maintain healthy and independent lives.


Initial Assessment by a Social Worker
Recommendations and Referrals of Services
Regular Visits and ongoing Re-evaluation
Assistance in Communication with Primary Care Physician and other health care providers


Must be 60 and above with a chronic illness
