Community Care Network of Kin On

The Community Care Network of Kin On is an organization created to address the needs of two specific groups of community members: the elderly and disabled who wish to retain their independent lifestyle in their own homes; and the relatives and friends who help take care of them.

Our comprehensive network of programs provide practical, one-on-one assistance while meeting the cultural and linguistic needs of our clients. There is no charge for enrollment into our Network. All services are provided by our bilingual and bicultural staff. Chinese dialects spoken include, but not limited to Cantonese, Mandarin and Toishanese.

Home Care

Kin On Home Care is licensed by the Washington Department of Health to provide home care services in King County.  This program provides assistance with personal hygiene, daily living tasks, and household chores for physically challenged and chronically ill clients in an environment they are most familiar with — their own home. 

Home Care services include: 

  • Personal Hygiene: Assistance with bathing, body care, and with dressing

  • Daily Living: Assistance with medication intake, essential shopping and travel to medical services, ambulation, transfer and positioning 

  • Housework: Assistance with dusting, laundry, linen changing and other services as needed

Payment Methods: 

  • Private Pay or State Funded Services with a two-hour minimum.

  • State funded services include: COPES Personal Care, Medicaid Personal Care, and In-Home Respite

Home Health

Kin On Home Health provides nursing, rehabilitation and  other professional services to patients and their families in partnership with Evergreen Home and Community Services, Visiting Nurse Services of North-west, and other home health agencies. These services are coordinated with the patient’s physicians to assist patients in recovering from surgery, illness, or accidents. Eligibility for this program is determined by the patient’s health condition and insurance coverage.

Home Health services include: 

  • Registered Nursing Care (English, Chinese, Korean and Japanese)

  • Physical, Occupational and Speech Therapy

  • Social Services

  • Personal care by Home Health Aides 

Payment Methods: 

  • Medicare 

  • Medicaid

  • Private insurance

Hospice Services  

Partnering with Evergreen Hospice Services, the Kin On Hospice Care program provides medical, emotional, and spiritual comfort care for those with life-limiting illnesses and their families.  With a culturally and linguistically sensitive, team-based approach, patients and families are supported throughout the life-limiting illness or the loss of a loved one.

Hospice care is for those with a life-limiting illness, with a prognosis of six months or less, whose desired focus of care is on comfort and quality of life.  To experience the full benefit of hospice care, we encourage patients, families, and their loved ones to inquire as early as possible - ideally months before the illness runs its course.

Services Include:

  • Hospice Medical Services provided by Medical Doctors, Registered Nurses, Medical Social Workers, Pharmacists and other healthcare professionals

  • Oxygen, Medical Equipment and Supplies available to use at home

  • Certified Nursing Assistants to assist with personal care

  • Trained Volunteers to offer companionship for patients and respite for families

  • Spiritual Care to offer spiritual support

  • Bereavement Services available after the loss of a loved one

  • Services are offered in English, Chinese and Korean

Family Caregiver Support

Caring for a family member can often be a daunting responsibility. In fact, research shows that family members who provide care to individuals with chronic and disabling conditions are themselves at risk for higher levels of depression, stress, frustration and heart disease. The Kin On Family Caregiver Support program is here to offer caregivers the support they need to successfully provide for their loved ones.

Services Include:

  • Needs assessment for caregivers and the care recipient 
  • Care planning and consultation 

  • Information and referral 

  • Caregiver education 

  • Alzheimer's and dementia support 

  • Respite care

Alzheimer's & Dementia Support

By adopting a culturally and linguistically sensitive approach, this program aims to enhance the community awareness of Alzheimer’s Disease and provide services to families with members who are inflicted with Alzheimer’s or other types of dementia.

Services Include:

  • Resource information 

  • Consultation 

  • Support and Counseling for families 

  • Referral to Clinical diagnosis 

  • Service referrals and setup 

  • Respite care services

Chronic Care Management

The Chronic Care Management Program is offered to people ages 60 or above with chronic diseases. According to the U.S. National Center for Health Statistics, a chronic disease is a disease which lasts for more than three months.

Based on thorough and ongoing assessment of the client’s physical and mental needs, caregiver concerns and financial situation, the Kin On Chronic Care Management Staff will provide you and your family the support you need to maintain healthy and independent lives.

The Chronic Care Management Program offers: 

  • Initial assessment by social worker 

  • Recommendations and referrals of services based on thorough assessment of client's physical functional level, medications, diagnoses, cognitive/mental functioning, caregiver concerns, financial concerns, etc. 

  • Regular visits and ongoing re-evaluation by social worker with clients, family members, caregivers and primary care physicians 

  • Assistance in contacting primary care physician or other health care providers

Personal Emergency Response System (PERS)

What is PERS?

Using the Lifeline System, PERS is an emergency response system that ensures that older adults living at home get quick assistance whenever it is needed – 24 hours a day, 365 days a year.

How does this work?

  1. When you need help, press the waterproof Personal Help Button you wear as a pendant or wristband.
  2. Within seconds, a certified Lifeline Monitor responds in your language and assesses the situation.
  3. The Lifeline Monitor contacts a friend, neighbor, loved one or emergency services to come to your assistance.

What is Lifeline Reminder?

With the Lifeline Reminder feature, caregivers can record up to six personalized reminder messages and play them on a regular schedule. Reminders are perfect for...

  • Taking medication on time
  • Remembering medical appointments
  • Following your doctor’s guidelines
  • Remembering important dates like birthdays and anniversaries

How much does the service cost?

You can receive this service for a little more than a dollar a day with no long-term commitment required.  Kin On accepts private pay or state funded-COPES Personal Care (if eligible).

Kin On Resource Library

Following the principal to serve the public and to actively promote community education, Kin On has opened a library of resources on medical and health issues for the public to use. 

Our Alzheimer's and Dementia Resource Center also carries educational pamphlets, booklets, reference materials, audio and video cassette tapes. All resources are available in Chinese and English and some have been translated into several Southeast Asian languages. 

If you are interested in learning more about medical and health matters, have questions regarding your health, currently caring for persons with disability or chronic illnesses, or know someone who is a caregiver, we urge you to come visit our library! 

The Kin On Resource Library is open on Monday to Friday, 8:30am to 5:00pm.
